Bear Lake Memorial Healthcare is seeking a compassionate and skilled Registered Nurse to join our team, dedicated to providing high-quality, patient-centered care and supporting effective case management, care coordination, and discharge planning.
Essential duties include, but are not limited to:
· Maintain accurate, complete, and timely patient records in accordance with facility policies, regulatory standards, and professional guidelines.
· Ensure compliance with all applicable regulations, hospital policies, and accreditation requirements.
· Communicate effectively with physicians, patients, families, visitors, and interdisciplinary team members to support coordinated, patient-centered care.
· Assist patients and families with questions related to care, treatment, and case management services.
· Support case management functions, including coordinating services and facilitating continuity of care between departments, physicians, other facilities, patients & family, etc.
· Monitor all inpatients to identify discharge planning needs and assist with appropriate and timely discharge coordination.
· Perform utilization review activities to ensure appropriate level of care and documentation during inpatient stays.
· Assist with submission of required clinical documentation to insurance providers.
· Complete the PASRR (Preadmission Screening and Resident Review) processes as needed.
· Conduct preoperative patient interviews and assessments.
· Formulate a discharge plan for each admission, starting on the day of admission and document in the nursing care plan.
· Assist the patient with obtaining equipment, aids, etc. and in utilizing the community resources best adapted to meet his/her needs upon discharge.
· Verify patient demographic information, insurance coverage, and current medication lists.
· Coordinate with physicians to ensure required preoperative labs and testing are completed within appropriate timeframes.
· Review surgical documentation to ensure boarding slips, history and physical (H&P), and consent forms are complete and consistent.
· Monitor surgery schedules and review notifications received via email or fax from the OR scheduler.
· Review and respond to communications related to surgical scheduling and case coordination.
· Perform other related duties as assigned or required to support patient care and departmental operations.
Requirements:
· Consistently work cooperatively with patients, residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers.
· Consistently supports compliance and the principles of responsibility by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licensure requirements (if applicable), and Bear Lake Memorial’s policies and procedures.
· Knowledgeable of hospital services.
· Must be able to sit for long periods of time.
· Must be capable and experienced with a computer.
· Good use of grammar and spelling.
· Experience with MSWord and Excel.
· Must be able to do a minimal amount of lifting, bending, twisting, etc.
· Willing and able to be flexible and to work various hours/days.
· Excellent service and communication skills when interacting with employees, family members and visitors.
· Must be kind and compassionate.
· Very good organizational skills.
